- USPSTF Issues Grade D for PSA screening sends the wrong message. The US Preventitive Services Task Force (USPSTF) issues a grade of D for using PSA in screeing for prostate cancer. To abolish the PSA level for screening would regress back to digital rectal exams prior to the PSA era. SEER survival statistics from the National Cancer Institute Table 23.9 shows a clear survival benefit in early detection. The PSA test was accepted as a screening tool around 1990, beginning the PSA era. The survival rates for prostate cancer patients has dramatically reached upwards of 99% in the last decade. The American Urologic Association issues a strong opposition to the USPSTF recommendation and is gathering a panel of experts to submit comments to the USPSTF.Learn more from our latest Newsletter.
- FDA Issued Advisory on Transvaginal Mesh Repair for Pelvic Organ Prolapse and Related ComplicationsThe FDA recently issued an advisory on using transvaginal mesh to repair pelvic organ prolapse due to a perceived increase in complications. As an expert in pelvic floor reconstruction and a valued leader in the field, Dr. Shawn Blick recognizes the events that have led to the FDA's report, and he agrees with many of the points covered in the FDA's Safety Communication. However, Dr. Blick strongly believes that the recent FDA UPDATE fails to convey an accurate perspective to the public, to the press, and unfortunately, to the legal community. He also feels that several key conclusions in the UPDATE are not consistent with the scientific literature pertaining to vaginal mesh and are inconsistent with the clinical realities that we encounter as surgeons caring for women with severe prolapse and incontinence.
- FDA Issues Reccomendations on Actos to Patients with Bladder CancerThe FDA issues a drug update in regards to ongoing post market research. The five-year results showed that although there was no overall increased risk of bladder cancer with pioglitazone use, an increased risk of bladder cancer was noted among patients with the longest exposure to pioglitazone, and in those exposed to the highest cumulative dose of pioglitazone.
FDA is also aware of a recent epidemiological study conducted in France which suggests an increased risk of bladder cancer with pioglitazone. Based on the results of this study, France has suspended the use of pioglitazone and Germany has recommended not to start pioglitazone in new patients.
FDA recommends that healthcare professionals should:
Not use pioglitazone in patients with active bladder cancer.
Use pioglitazone with caution in patients with a prior history of bladder cancer. The benefits of blood sugar control with pioglitazone should be weighed against the unknown risks for cancer recurrence.
